AUG. 2

Red Salt, 12221 West Broad Street – The following violations were reported during a routine inspection: strainer pan found in bar hand sink (hand sinks are for handwashing only); mussels are on the menu but there are no recent shellstock tags available; raw beef incorrectly stored over tempura and raw fish incorrectly stored above sauces and carrots; there is 0 ppm chlorine sanitizer in dish machine in main kitchen; bar dish machine was at 10 ppm the first time it was tested; observed buildup inside ice machine; temperature of lobster soup made yesterday in walk-in is elevated because it was not cooled properly; elevated food temperatures found in a few make tables on the line; roasted red peppers and pimento cheese are not date marked; time isn’t marked for tempura on cook line; dropper bottle of clear liquid at bar is unlabeled; white powdery food in container on storage shelf and bulk container of vinegar are unlabeled; observed small flies near three-compartment sink; utensils at bar are stored in ambient temperature water; lexan pans are stacked while still wet on shelf; several cutting boards, particularly small ones at make tables, are heavily scratched; floor grout is worn in some areas, especially at cook line (water and food debris are pooling in the cracks); two lights in the hood are burned out.

Chipotle Mexican Grill, 8675 Quioccasin Road – The following violation was reported during a routine inspection: dish machine isn’t working.

Gojo Ethiopian Restaurant, 10188 West Broad Street – The following violations were reported during a routine inspection: paper towel dispenser at hand sink isn’t operational; prepared food items in reach-in cooler are not date marked; single cutting boards are heavily scratched; observed grease and food residue on wire racks, backsplash and hood filters.

AUG. 7

McDonald’s, 7118 West Broad Street – The following violations were reported during a routine inspection: an employee with painted nails is handling fries and not wearing gloves; several fryer baskets are in disrepair.

Tony’s Pizza Express, 8191-K Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; no handwash signage is posted at hand sink; observed buildup on slicer and inside dough pans; mold found in tops of sauce bottles; temperatures of chicken, lettuce, cheese and sour cream are elevated; time is not being tracked for pizza sold by the slice; cutting boards are stained and heavily scratched; multiple refrigeration units are in poor repair; did not observe plugs for three-compartment sink so it is unable to be filled with sanitizer; facility couldn’t provide a quat sanitizing test kit for three-compartment sink; observed dirt and debris buildup on racks, gaskets and inside units; observed grease buildup on fryers, fryer baskets and other equipment on cook line; observed blockage of pipes at three-compartment sink which is causing water to leak into a bucket; hood filters have grease and dust buildup.

La Vita è Bella, 11363 Nuckols Road – The following violations were reported during a routine inspection: observed employee drinking from an uncovered beverage mug in the kitchen and storing it on a cutting board; kitchen hand sink doesn’t have hand soap; sanitizer concentration in dish machine is too low; temperatures of deli turkey, ham and cheese in salad prep unit are elevated; this unit is not working properly; observed an employee wipe their hands on a cloth and then use the same cloth to wipe in-use knife and cutting board; prep cutting boards are heavily scratched; fryer baskets are in disrepair; wire racks in reach-in coolers are rusty.

Firebirds Wood Fired Grill, 11800 West Broad Street – The following violations were reported during a routine inspection: person in charge unable to provide documentation to verify that employees have received training on their illness reporting responsibilities; observed an employee use bare hands to transfer cooked mushrooms from a sheet pan to another container for storage; bar hand sink doesn’t have hand soap; sanitizer level at three-compartment sink in kitchen is too low; there is no sanitizer at the three-compartment sink at the bar and the bartender couldn’t find the sanitizer; commercially-processed mashed potatoes were not reheated to the required temperature; temperatures of garlic and Cajun butters out on the cook line are elevated; there are elevated temperatures in the make table stocked this morning (pork belly, goat cheese, rice and cooked wings); temperatures are marginal in the walk-in (raw flounder); observed many large flies in the restaurant; one live roach found crawling behind wall in kitchen; a wet wiping cloth was left out on the counter instead of being stored in a sanitizer bucket; utensils at cook line are stored in ambient temperature standing water; dishes are stacked while still wet on clean shelves in dish room; cutting boards are heavily scratched; observed leaks under cook line hand sink and three-compartment sink in kitchen; toilet in women’s restroom is out of order; there is grease buildup on floor under cooking equipment at the cook line, especially under fryers; wet mops are not being hung to air dry; floors are in poor repair; there are missing and damaged tiles, especially in the dishwashing area (a lot of standing water seeping into the floor there).

AUG. 9

Jersey Mike’s Subs, 8654 Staples Mill Road – The following violations were reported during a routine inspection: observed an employee fail to wash hands after sweeping floors and changing sanibuckets and before continuing food prep; sanitizer concentration in buckets is at 0 ppm; temperatures of tomatoes in front make unit and in top portion of prep unit are elevated; there is heavy buildup of ice on walk-in freezer floor; bread rack cover is torn; observed grease and food debris on portable fans, wire storage racks above three-compartment sink and prep tables, and in walk-in cooler; fan grate in cooler is dusty; mop sink doesn’t have a dual check valve installed; there are broken and missing floor tiles at back door; mops are not being hung to air dry; observed dirt and food debris on floor under mop sink and in walk-in cooler.

Starbucks inside Publix, 10150 Brook Road – No violations reported during a routine inspection.

AUG. 10

Yummy Chinese Food Inc., 6023 Nine Mile Road – The following violations were reported during a routine inspection: hand sink doesn’t have hand soap; some foods are incorrectly stored in walk-in cooler and reach-in cooler (raw chicken wings next to egg rolls and cabbage, raw fish over onions, raw chicken over noodles and produce, and raw chicken over egg rolls); observed food debris and rust on can opener blade and holder; egg rolls prepped today are not date marked; plastic to-go containers improperly used as scoops throughout the kitchen; wire racks throughout the kitchen are rusty; there are stained ceiling tiles throughout kitchen; walk-in cooler ceiling and fan guards are dusty.

Wok N Roll, 7514 West Broad Street – The following violation was reported during a routine inspection: temperatures of eggrolls and chicken in reach-in cooler are elevated (this unit isn’t working properly).
